In order to assist those who are not able to use a computer and/or do not have an internet connection, Coastal Kindness has replicated the online South African COVID-19 vaccination registration form on paper.
Over 60’s will be able to pick up a form from the Kenton Pharmacy or doctor’s rooms, complete the form manually and pop it in the box provided. A Coastal Kindness volunteer will collect and process the form on-line on their behalf.
The registrant will be notified by the National Department of Health Electronic Vaccination Data System by text message if the registration was successful.
If you know of someone who may find this service useful, please let them know – and possibly even deliver a form to them!
We hope to extend this service to surrounding areas if required.
